ALIA Health Care Program (Medicum / Maple)

Your Alia (Medicum) membership includes access to the benefits of the Alia / Maple health care program.

Medicum has entered into an agreement with Maple under which you, along with up to four family members, spouses or dependents, may access the health care services provided through our partner, Maple.

The health care services offered through Maple are provided by duly licensed health care professionals, including physicians, specialized nurse practitioners and other authorized providers practising in your province of residence. These services are delivered remotely in accordance with the terms and procedures set by Maple.

Medicum assumes no responsibility for the provision of the services made available through Maple and does not itself deliver these services. The terms of use governing Maple’s health care services are those established by Maple, and you will be required to consent to them when registering with Maple.

The benefits of the Alia / Maple health care program vary depending on your Alia membership option. There are two membership options: the Essentials plan or the Peace of Mind plan. The specific benefits associated with each option are described at: aliasante.ca/plans. Unless you choose the Peace of Mind plan, you will be enrolled in the Essentials plan by default and will remain enrolled in that plan as long as you are a member of the Alia program, or until you choose the Peace of Mind option.

You may, at any time and at your sole discretion, choose to change your membership from one option to another. This change will take effect on the first day of the month following the date on which you notify Medicum of your intention to make the change. The procedure for requesting this modification is available at the following address: [insert the web address where this information will be available]. Your membership fee will be adjusted accordingly.

In order to provide you with access to the Alia / Maple health care program services, Medicum must share certain personal information with Maple that you have provided to Medicum when enrolling in the Alia program. This information includes your first and last name, date of birth, and email address. If needed, Medicum may also provide Maple with your mobile phone number and your preferred language for spoken or written communication. By signing this form, you explicitly consent to the disclosure of the personal information described here.

Once your enrollment in the Alia program is complete, Medicum will inform Maple of your eligibility for the Maple health care program. You will then be invited to register directly with Maple using the identifier provided to you by Medicum. Medicum is not an agent, representative, partner or subcontractor of Maple. Medicum is not a party to your relationship with Maple, except for facilitating your enrollment in the Maple health care program.

Personal information that you choose to provide to Maple, or that may be generated in the course of the health care services you receive through Maple, will only be shared with Medicum if you explicitly consent to such sharing through Maple. If such information is transmitted to Medicum, it will be added to the record maintained in your name under the Alia program and retained in accordance with Medicum’s personal information confidentiality policies and all applicable laws.
